I, hereby grants and authorize, PSI ZETA PHI MILITARYSORORITY, INC the unrestricted right to take, edit, alter, publish, post and make use of all photos, videos and/or audio ("the materials") that have been taken of me. I further grant permission for the materials to be used for any lawful purpose, including but not limited to advertising, promotion, marketing, and packaging, without payment or consideration.*

I understand that PSI ZETA PHI MILITARY SORORITY,INC may modify or edit the materials at theirdiscretion, and I waive any right to inspect or approve the final images or any finished product where thesematerials may be used.*

I acknowledge that I will not receive any financial compensation for the use of these materials, and that all rights, title, and interest in the images belong to PSI ZETA PHI MILITARY SORORITY, INC. I further release PSI ZETA PHI MILITARY SORORITY, INC from any claims, demands, or liabilities arising out of or in connection with the use of these materials, including but not limited to any claims for defamation, invasion of privacy, or copyright infringement*

I understand that PZI ZETA PHI MILITARY SORORITY, INC holds the ownership and copyright to the materials mentioned above and has the exclusive right to reproduce, distribute, display, and create derivative works based on these images. I agree not to reproduce or authorize the reproduction of these images in any form without the express written consent of PSI ZETA PHI MILITARY SORORITY, INC*

This permission shall continue indefinitely and applies to all anguages, media formats, and markets.

I hereby release and hold harmless PSI ZETA PHI MILITARY SORORITY, INC, and its agents, representatives, from any and all claims or actions arising out of, or in connection with the use of the materials.

I confirm that I am over the age of 18 and have read and understood the terms of this release form. I also affirm that I have the legal authority to grant the permissions and rights stated herein.
